- The electrocardiogram (ECG) is one of the most important investigations in drug overdose because it provides information on cardiac arrhythmias and the risk of arrhythmias occurring secondary to drug toxicity. Although cardiac arrhythmias are rare they are one of the most important life-threatening complications of drug overdose. For this reason predicting the probability of an arrhythmia occurring for particular drugs in overdose is of paramount importance in clinical toxicology. The key to such a risk assessment is identifying features on the ECG that predict malignant arrhythmias, such as QRS widening in tricyclic antidepressant poisoning and early features of digoxin toxicity. QT interval prolongation is another important measure on an ECG that is associated with, and therefore can predict, the occurrence of Torsade de pointes (TdP).
